Appointment Request

Thank you for your interest in our practice, here are some things you should know so we can better assist you.  On average we are booking about 2-3 weeks out for routine appointments, please select a few dates that are a few weeks out when possible.  If your pet is suffering from an illness or injury and you feel that it needs to be addressed sooner, please call the office at 269-948-2222.  We are accepting new clients, a deposit is required to schedule the initial visit.  PLEASE NOTE: THIS IS AN APPOINTMENT REQUEST FORM.  Once we receive the request, one of our receptionists will reach out to you to discuss the appointment and get something scheduled.
